Life-changing events like Marriage, Divorce, Having a Child, or Loss of Coverage allows the option to enroll outside of Open Enrollment.

What to Bring to Enroll

Identification

You will need the Name, Date of Birth, and Social Security Numbers of everyone who will be applying for coverage.

Proof of Income

Please bring proof of income for every household member who works.   Examples of proof of income: Tax Return, Check Stubs, or Social Security Benefits

Immigration Status

The most common eligible immigration documentation includes: Permanent Resident Card, “Green Card”, Employment Authorization Card (I-766), or Reentry permit (I-327)
