Payment Options:

At Amistad, we accept most major insurance plans.  Questions about your specific plan?  Call us at 361-884-2242, Ext. 0.

For patients without health insurance coverage, we offer a sliding fee scale based on the Federally established guidelines of family size and income to determine the cost of services provided.

Texans can apply for these state benefits programs at Amistad:

  • Health care assistance through Medicaid
  • Children's Health Insurance Program (CHIP)
  • Healthy Texas Women (HTW)

What To Bring

Please bring your photo ID and insurance card to every appointment

  1. Two forms of identification
  2. Proof of income- Your most recent income tax return or check stubs from your most recent paycheck
  1. Photo identification
  2. Social Security card
  3. Income tax return
  4. Income verification
  5. List of your medications
  1. Birth certificates and Social Security cards for all children in the household
  2. Proof of residency listing the members of the household living at that address
  3. Proof of income- Your most recent income tax return or check stubs from your most recent paycheck
