For patients without health insurance, we offer a sliding fee scale based on family size and income. This helps determine the cost of services according to Federally established guidelines.

  2. Proof of income- Your most recent income tax return or check stubs from your most recent paycheck

  2. Social Security card
  3. Income tax return
  4. Income verification
  5. List of your medications
  1. Birth certificates and Social Security cards for all children in the household
  2. Proof of residency listing the members of the household living at that address
  3. Proof of income- Your most recent income tax return or check stubs from your most recent paycheck
