Quality Care Incentive Programs Manager

Salary: $65,000 – $75,000 per year

We are seeking a skilled and motivated Quality Care Incentive Programs Manager to support Amistad’s value-based care and population health initiatives. This role focuses on quality performance, financial tracking, care gap improvement, reporting requirements, and contract management across all service lines and locations.

Key Responsibilities

  • Manage value-based care and payer incentive contracts
  • Track quality performance, care gaps, and financial outcomes
  • Support UDS, payer, and CMS reporting requirements
  • Ensure coding and billing accuracy for population health programs
  • Provide data analysis to guide improvement efforts
  • Collaborate with CMO/COO, CFO, and clinic leadership
  • Support HRSA and FTCA compliance standards

Requirements

  • Bachelor’s degree required (Master’s preferred)
  • 3–5 years of experience in population health or managed care
  • Strong communication, leadership, and analytical skills
  • Proficiency in EHR, population health software, and Microsoft Office

Nurse Practitioner

We are seeking a compassionate and experienced Nurse Practitioner to provide high-quality primary care services to patients of all ages while supporting Amistad’s mission of accessible, patient-centered care.

Key Responsibilities

  • Provide comprehensive primary care services
  • Manage acute and chronic conditions
  • Prescribe medications and order diagnostic tests
  • Document patient care accurately in the EHR
  • Collaborate with physicians and care teams
  • Participate in quality improvement initiatives
  • Ensure compliance with clinical standards

Requirements

  • Graduate of an accredited NP program (MSN or DNP)
  • Current Texas NP license
  • National board certification (FNP preferred)
  • DEA license or ability to obtain
  • Experience in primary care or community health preferred

Manager, Managed Care (RN)

Reports To: CMO

Salary: $80,000 – $90,000 per year

The Manager, Managed Care (RN) leads population health and managed care initiatives, overseeing high-risk patient care management and quality performance to improve outcomes and meet organizational goals at Amistad Health.

Key Responsibilities

  • Define and manage clinical standards and protocols under supervision of the CMO
  • Develop and oversee care coordination and quality improvement programs
  • Identify, enroll, and manage high-risk patient populations
  • Coordinate transitions of care and post-discharge follow-up
  • Support medication management and 340B pharmacy initiatives
  • Collaborate with Finance, Billing, Informatics, and multidisciplinary teams
  • Ensure compliance with Amistad policies, procedures, and quality standards

Requirements

  • Bachelor’s degree in Nursing; current Texas RN license; BLS/CPR
  • Minimum of five (5) years RN experience in managed care or primary care
  • Strong knowledge of value-based care, HEDIS, UDS, and care coordination
  • Leadership, communication, and organizational skills
  • Bilingual English/Spanish preferred
