Quality Care Incentive Programs Manager

Salary: $65,000 – $75,000 per year

We are seeking a skilled and motivated Quality Care Incentive Programs Manager to support Amistad’s value-based care and population health initiatives. This role focuses on quality performance, financial tracking, care gap improvement, reporting requirements, and contract management across all service lines and locations.

Key Responsibilities

  • Manage value-based care and payer incentive contracts
  • Track quality performance, care gaps, and financial outcomes
  • Support UDS, payer, and CMS reporting requirements
  • Ensure coding and billing accuracy for population health programs
  • Provide data analysis to guide improvement efforts
  • Collaborate with CMO/COO, CFO, and clinic leadership
  • Support HRSA and FTCA compliance standards

Requirements

  • Bachelor’s degree required (Master’s preferred)
  • 3–5 years of experience in population health or managed care
  • Strong communication, leadership, and analytical skills
  • Proficiency in EHR, population health software, and Microsoft Office

Nurse Practitioner

We are seeking a compassionate and experienced Nurse Practitioner to provide high-quality primary care services to patients of all ages. This role supports Amistad’s mission of delivering accessible, patient-centered care while working collaboratively with physicians and clinical staff.

Key Responsibilities

  • Provide comprehensive primary care services including assessment, diagnosis, and treatment
  • Manage acute and chronic conditions across the lifespan
  • Prescribe medications and order diagnostic tests as appropriate
  • Document patient care accurately in the EHR
  • Collaborate with physicians, nurses, and care team members
  • Participate in quality improvement and population health initiatives
  • Ensure compliance with clinical guidelines and regulatory standards

Requirements

  • Graduate of an accredited Nurse Practitioner program (MSN or DNP)
  • Current Nurse Practitioner license in the State of Texas
  • National board certification (FNP preferred)
  • DEA license or ability to obtain
  • Experience in primary care or community health preferred
  • Strong clinical, communication, and documentation skills
  • Comfort working in a fast-paced, team-based environment
